Social Services assists individuals in choosing the most appropriate services to meet their needs and to empower the client to maintain an independent and self-directed lifestyle.

A key role of Social Services is to identify needs with the client, take action to work toward resolving issues and arrange for the provision of appropriate services.

Social Services works closely with the local community and other interested parties to effectively target and deliver services where they are most needed.

Services Offered

Group Services

Wellness is encouraged through educational and recreational venues.  Activities offered include creative arts, health promotion, disease prevention, and socialization.  Programs at the Oak & Acorn Intergenerational Center focus on relationships with persons of all ages.

 

 

Individual Services

This social services program meets a variety of individual needs, such as assistance in finding resources to help with everything from post-hospital plans and housing to transportation, government benefits and family relationships.
